The ball hit him in both hands. He simply flubbed the shot.

Both teams played well, actually. The only goals were because of brief ‘errors’. First on the part of a US defender that misstepped, and gave Gerrard an ‘easy’ (relatively speaking) shot on the goal. The US goal was because Green made an error in timing.
